Tuberculosis is an infectious disease caused by the Mycobacterium tuberculosis bacterium. It is, after SARS-CoV-2, the second leading cause of death worldwide from an infectious agent. Humanity has coexisted with the tuberculosis bacterium for thousands of years, and over time, both the bacterium and humans have evolved to persist together. Proof of this is the strong genetic basis that gives us resistance to M. Tuberculosis infection, where more than 90% of infected individuals remain asymptomatic. This is the first study in our country that evaluates susceptibility to TB considering xenobiotic metabolism genes such as NAT2. We have sought to establish the relationship between the rs1799929, rs1799930, and rs1799931 polymorphisms in the NAT2 gene and the progression of tuberculosis in Peruvian patients through an association analysis between our group of 109 TB patients, obtained from the Guillermo Almenara Hospital, in contrast with a control group of 85 healthy Peruvians. A significant association was found for the rs1799931 variant and the development of tuberculosis, as well as an association between the slow and intermediate acetylator phenotype for NAT2 and tuberculosis patients. Without neglecting that susceptibility to tuberculosis is multigenic, we can deduce that in the Peruvian population, the rs1799931 variant, which is more common compared to other countries, increases the probability of developing tuberculosis in our population.
