Weaning from mechanical ventilation (MV) can be successful or result in failure. With the objective of evaluating the usefulness of a protocol to predict successful weaning from MV, a study was carried out in a retrospective cohort of 92 patients with ARDS due to COVID-19 and MV for more than 24 hours. This protocol used 15 criteria, including resolution of the cause of MV, PEEP ≤8cmH2O, PaO2/FiO2 ≥150mmHg, FiO2 ≤40%, hemodynamic stability, normal serum potassium and phosphorus, successful SBT, cuff leak test negative, adequate level of consciousness, cough and swallowing, poor secretions, and use of NIV or HFNC post-extubation. The utility of the protocol is based on its positive predictive value and specificity, given that the probability of successful weaning would be 100% if the protocol is complied with in its entirety; likewise, 100% of patients with failure as a result would not have complied with the protocol completely.
