Introduction: Patients with cholecystitis develop various inflammation alterations at the level of the gallbladder. The Parkland grading scale has been developed for intraoperative evaluation of the anatomic status and inflammation of the gallbladder. Our objective: was to evaluate the application of the Parkland classification scale in patients with cholecystitis to identify the severity of gallbladder inflammation. Materials and Methods: Cross-sectional, temporally retrospective, descriptive observational study, one-way ANOVA and the Chi square test were used to evaluate Parkland classification degrees with preoperative, perioperative and postoperative measures in all patients with cholecystitis. Results. Among 108 patients who underwent cholecystectomy, there were 33 (grade 1), 42 (grade 2), 17 (grade 3), 9 (grade 4), and 7 (grade 5). The values of preoperative leukocytes, C-reactive protein, gallbladder wall thickness, the percentage of conversion to open cholecystectomy, as well as the average operative time and hospital stay increased significantly with increasing grades of the classification scale. Parkland (p < 0.0001). Conclusion. This means that the inflammation of the gallbladder and the difficulty for the surgical procedure is well established as the degree of severity increases and therefore the application of Parkland classification scale in patients with cholecystitis allows evaluating the severity of inflammation of the gallbladder. gallbladder to predict the intraoperative and postoperative course in laparoscopic cholecystectomy.
