Introduction: Hematological alterations are causes of deferral in postulants to donate blood and only the measurement of hemoglobin is not enough for its detection. Basic knowledge about the frequency of these alterations could help to generate strategies for donor selection. Objective: to determine the frequency of alterations in hematological parameters in applicants to donate blood. Materials and Methods: Cross-sectional study carried out at the Hemotherapy and Blood Bank Service of the Cayetano Heredia Hospital during August 2022 based on the review of medical records of 610 applicants to donate blood. Demographic data (age and sex) and hemograms were reviewed and performed in the automated hematological analyzer CELL-DYN Emerald System (Abbott Diagnostics). Results: The age range of 29 to 39 years was the most representative, being predominantly male (64.2%). The frequency of hematological alterations was 27.2%. Macrocytosis was the most common finding in the red series (14%). Anemia was more frequent in women (84.6%), being microcytic and hypochromic microcytic anemia the most observed findings in this group. Thrombocytopenia and leukocytosis were more common in males with 83.3% and 61% respectively. Conclusions: Our study describes a high frequency of hematological abnormalities in postulants to donate blood. The use of hemoglobin alone is not sufficient to establish the absence of hematologic abnormalities in blood donors
