Introduction: Emergency abdominal surgery increases mortality 8 times in relation with patients who undergo the same intervention electively. Some authors suggest that 15 to 27% of patients admitted to the hospital for this condition receive postoperative management in the intensive care unit (ICU) and 21.2% die during their stay there. In Peru, there are no articles that describe mortality, postoperative characteristics, and admission to intensive care in patients who require this type of critical management. Main study objective: Determine the mortality of patients undergoing emergency abdominal surgery during their postoperative stay in the intensive care unit of Hospital Cayetano Heredia. Materials and methods: Exploratory retrospective longitudinal case series. Electronic medical records of patients who met the inclusion and exclusion criteria were reviewed from January 1, 2021 to March 31, 2023. Results: 135 patients were found with a mortality of 19.2% within the ICU, and 27.4% at the hospital level, the most frequent pathology in these patients was biliary (24.4%) and the highest mortality per procedure was that of laparotomy. exploratory + small bowel resection (40%). The laboratory values of these patients, use of vasopressors, use of invasive mechanical ventilation and ICU length of stay, were satisfactorily described, finding no significant differences between living and deceased patients. However, age, Charlson comorbidity index, SOFA score, APACHE II score and norepinephrine dose were significantly higher in patients who died during their ICU stay. Conclusion: Despite the limited resources at Hospital Cayetano Heredia, a mortality rate of 19.2% is maintained, according to what is described in the literature.
