Background: The worldwide incidence of intraventricular hemorrhage (IVH) has decreased to 25-30% while in Peru it can reach 15-50%. In our environment the decrease is dissimilar between health institutions, being of interest its associated factors for prevention and control. Objectives: To describe the annual incidence of IVH between 2018-2022 and to evaluate the association between the use of early CPAP and CPR with IVH in premature infants ≤1500 g in a tertiary hospital. Material and method: Retrospective cohort registered at the base of the NEOCOSUR Network, with transfontanellar ultrasound. Neonates with congenital malformations and incomplete data for IVH were excluded. Results: We identified 344 premature infants and 288 were included, 56 were excluded because they did not meet criteria, due to gestational ages greater than 33 or less than 23 weeks. The frequency of IVH was higher in 2018 (45.95%) and presented a slow decrease. The multivariate analysis identified the use of CPAP as a protective factor [ORa:0.34;(IC:0.16-0.74);p=0.006] independent of: gestational age, number of surfactant doses, antenatal corticosteroid doses, Apgar score at 5', advanced CPR, early sepsis, hemodynamically significant PDA, type of delivery, mechanical ventilation in the 1st week and chorioamnionitis. We found no association with CPR [ORa:0.99;(IC:0.51–1.96);p=0.989]. Conclusions: IVH is frequent and higher than the worldwide incidence. The use of CPAP protects against the development of IVH while CPR, of any type or advanced, is not associated with this pathology in our population.
