Introduction: Insulin resistance (IR) could originate in the first years of life. We used relative leg length as a biomarker of childhood malnutrition to assess its association with IR in rural, urban, and rural-urban migrant settings. Objective: To determine the association between relative leg length and insulin resistance (IR) in a rural, urban and rural-urban migrant population of Peruvian adults. Methods: Cross-sectional study, analysis of secondary data from the PERU MIGRANT study. The RI was measured by the logarithm of HOMA-IR. Relative leg length was measured by dividing leg length by height, standardized to Z-scores for sex and age, and categorized as short, normal, and long. We used multiple linear regression models adjusted for sex, elderly, parental educational level, hip circumference, and physical activity, with robust estimates of variance, and explored the interaction of population groups with relative leg length categories in their association with IR. Finally, we analyze the mediation of central obesity and excess body fat in the association initially evaluated. Results: We analyzed data from 947 participants, 52.7% women, mean age 47.7 years (SD: 11.9). Participants in the “long” relative leg length category had 43% (95% CI: 31-53%) less IR than those in the “normal” category, being this association statistically significant (p<0.001). No differences in IR were observed between the participants in the “normal” and “short” categories (p=0.747). The population groups interacted with the relative length of the legs in their association on the IR, verifying a greater effect of the interaction in the group of rural-urban migrants (p<0.001). Excess body fat and abdominal obesity explained 33% and 12% of the association between relative leg length and IR, respectively. Conclusion: A "long" relative leg length was associated with lower IR values, observing a greater effect of the interaction with the rural-urban migrant group. This suggests that metabolic disorders may originate from the interaction between nutritional conditions in early and late stages of life.
