Background: It has been described that the prevalence of anemia in children has been overestimated. However, studies according to different definitions of anemia, altitudes of residence and ages are limited. Objective: To estimate the prevalence of anemia, during the period 2009-2021, in the age groups 6-35 and 36-59 months, at national and departmental level and by altitude of residence. Materials and methods: Analytical cross-sectional study, based on the Demographic and Family Health Survey. Three definitions of anemia were used: hemoglobin adjusted and unadjusted for altitude (Hb<11g/dL), and Hb<percentile 5 of children without chronic malnutrition. Results: In the period 2009-2021, the prevalence of anemia in the 6-35 months age group was 43.6% according to adjusted hemoglobin, 29.3% according to unadjusted hemoglobin and 5.2% according to the 5th percentile, and for the 36-59 months group, it was 19.3% according to adjusted hemoglobin, 10.9% according to unadjusted hemoglobin and 4.9% according to the 5th percentile. Prevalence according to adjusted hemoglobin increased gradually with altitude. Departments with the highest prevalence belonged to the rainforest (Madre de Dios, Loreto, Ucayali) and to the highlands (Puno, Cusco, Huancavelica). Prevalence according to adjusted hemoglobin showed significant correlations with chronic malnutrition, human development index, poverty and rurality. Prevalence according to unadjusted hemoglobin and 5th percentile presented inconsistent correlations. Conclusions: The definition of anemia has a great impact on its prevalence, with the prevalence according to adjusted hemoglobin being higher. Our results suggest that the prevalence of anemia according to adjusted hemoglobin at high altitudes could be overestimated.
