Multiple evidences demonstrate the frequency of cerebral lesions in the preterm infants that determine their survival and later development. ln our country the magnitude of this problem is not known. This is a prospective study which objective was to determine the incidence of the cerebral lesions, its associated factors and the prognostic in the premature below 34 weeks of gestational age. The investigated lesions caused by intraventricular haemorrhage, periventricular leucomalacia and hypoxic-ischemic encephalopathy, with serial ultrasonography. From 1999 to 2001, a hundred fifty three premature were selected, of which 64% present some type of cerebral lesion, 44% for intraventricular haemorrhage, 19.6% for periventricular leucomalacia and 15.7% for hypoxic-ischemic encephalopathy. The multivariate analysis determined that the factors related with the intraventricular haemorrhage were neonatal sepsis (p=0.016), membrane hyaline (p=0.008), patent ductus arteriosus (p=0.006), and the neumothorax. The multiparity (p=0.022) was the only factor associated with leucomalacia periventricular. Chorioamnionitis (p=0.009), podalic presentation (p=0.014), neumothorax (p=0.002) and renal insufficiency (p=0.008) were associated for hypoxic-ischemic encephalopathy. The mortality was 45.1%, 61 preterm infants had posterior control, from them, 29 babies had cerebral palsy, 28 had language retard and 5 had seizures. In conclusion, cerebral lesions are a frequent problem in the preterm below 34 weeks, they have high mortality, associated to several factors, and future handicap. There was a high incidence of intraventricular haemorrhage. These children require soon identification and control in the units of intensive care and in ambulatory vigilance.
