Introduction: High blood pressure (HBP) is one of the main cardiovascular risk factors. Since it is modifiable, several studies have evaluated its association with anthropometric characteristics related to adiposity, such as body mass index. However, these have limitations because they do not discriminate between the lean mass and the body fat mass of the individual. The Fat Mass Index (FMI) has been little studied and could be a more accurate indicator of adiposity. Objectives: To determine if there is an association between FMI and systolic and diastolic blood pressure and, additionally, to assess whether the evaluation over time (three measurements separated by approximately 15 months each), the place of residence or sex modify the association of interest. Methods: Secondary analysis of the CRONICAS Cohort Study that had three evaluations and was conducted in four settings in Peru. Participants with complete baseline blood pressure and FMI data were selected and each contributed with one or more observations (depending on the number of follow-up assessments they participated in) to the main analysis. Additionally, a sensitivity analysis was carried out excluding people with treatment for HBP. The FMI was categorized into quintiles and crude and adjusted mixed linear regression models for blood pressure with random intercept were generated for each individual. To assess effect modification, an interaction term was introduced to the fitted models. Results and Conclusions: For the main analysis, data from 3,217 participants with a mean age of 55.7 ± 12.7 years were included, 51.5% being female, providing a total of 8,729 observations accumulated in three visits over a period of 29.4 months. In a multivariate model, evidence of a strong association between systolic and diastolic blood pressure and FMI quintiles was found. Compared with quintile 1 (less FMI), a gradient of higher average mm Hg units was observed according to the higher FMI quintile, in a range of 2 mm Hg (Q2 vs Q1) to 9 mm Hg (Q5 vs Q1). The moment of evaluation modified the association of interest, with greater differences observed in some of the FMI quintiles at the first follow-up compared to the quintile 1 when compared to the baseline evaluation. However, when visually examining these results, no marked variation in the slopes is observed, indicating inconclusive findings.
