Background: HTLV-1 infection is endemic in Peru, but at the moment there are no studies on the knowledge and attitudes of Peruvian women with HTLV-1 regarding their infection and transmission through breastfeeding. Objective: To evaluate the knowledge and attitudes in women who live with HTLV-1 and attend an outpatient clinic in Lima, regarding their infection and breastfeeding. Methods: Cross-sectional descriptive study in women who attended consultation between September and December 2022 in a specialized Institute. Survey created with experts, including questions of knowledge about infection, breastfeeding and attitudes towards those topics. On-site enrollment prior to consultation. Results: 45 participants with a median age of 45 (IQR 36-55) were surveyed, 78% from metropolitan Lima, 69% who completed high school, and 49% reported suffering from a disease associated with HTLV-1. 93% correctly identified breastfeeding as a route of transmission, and 78% knew the currently accepted recommendations in this regard (World Health Organization). Only 26% had a diagnosis prior to any pregnancy. There was a positive attitude towards breastfeeding in 98%. 26% pointed to the need for better health systems. Conclusion: Participants exhibited good knowledge about HTLV-1 and breastfeeding, especially regarding transmission routes. The attitude towards breastfeeding in general and official recommendations about it is positive. Participants suggested improvements in public health policies and a need to broaden the knowledge of the population and health personnel on HTLV-1.
