Resumen: Introducción: La información sobre si los niveles de vitamina B12 influyen sobre la grasa corporal es limitada, especialmente en adultos no diabéticos no obesos. Objetivo: Determinar la asociación entre vitamina B12 y grasa corporal total en adultos no diabéticos y no obesos. Métodos: Estudio transversal analítico de pacientes ambulatorios de 18-59 años de una clínica privada, sin comorbilidades e índice de masa corporal de 18.5-29.9 Kg/m2. La exposición, vitamina B12 en sangre en ayunas, fue categorizada en terciles y el primer tercil fue clasificado como nivel disminuido. El desenlace, grasa corporal total por bioimpedancia, fue categorizada en normal y elevada. Se calcularon razones de prevalencia (RP) usando regresión de Poisson ajustada por edad, sexo y ferritina. Además, se evaluó su interacción con sobrepeso, sexo, y resistencia a la insulina. Resultados: 794 pacientes (88.8% mujeres y 69.4% de 18-39 años) fueron analizados. Casi la mitad tenía sobrepeso. Se encontró asociación entre vitamina B12 disminuida y grasa corporal total elevada (RPa: 1.13; IC95%: 1.02 a 1.25), sin observar interacción entre subgrupos evaluados. Conclusión: La vitamina B12 disminuida se asoció con grasa corporal total elevada en adultos no diabéticos no obesos, otorgando evidencia a favor de su relevancia sobre el metabolismo de lípidos.
Introduction: The knowledge on whether vitamin B12 influences body fat or not is still limited, especially in non-obese and non-diabetic adults. Objective: To assess the relationship between serum vitamin B12 and total body fat in non-diabetic and non-obese adults. Methods: We performed a cross-sectional study on outpatient adults 18-69 years old from a private clinic, with body mass index of 18.5-29.9 Kg/m2, and without known comorbidities. Exposure variable, fasting serum vitamin B12 levels, were dichotomized in decreased (lower tertile) and normal levels (second and third tertile). Sex-adjusted total body fat, measured by bioimpedance, was categorized as normal or high. Adjusted prevalence ratios (aPR) were estimated using Poisson regression models including the confounding variables sex, age, and ferritin levels. Interaction was tested for sex, overweight, and insulin-resistance groups. Results: 794 patients (88.8% females and 69.4% with 18-39 years old) were included. Almost half of them were overweight. We found an association between decreased vitamin B12 and high total body fat (aPR: 1.13, 95%CI: 1.02 to 1.25), without evidence of interaction between the assessed groups. Conclusions: Decreased serum vitamin B12 was associated with high total body fat in non-obese and non-diabetic adults, providing evidence in favor of its relevance to lipids’ metabolism.
