Background: Complications of diabetes mellitus can be avoided if the disease remains under control. But, previous studies in South America showed that the treatment and control of diabetes are inadequate. Objective: Determine the proportion of patients with diabetes that are aware of their disease, treated and controlled in South America. Methods: We conducted a systematic review with meta-analysis. Our search was carried out in Medline, SCOPUS and LILACS, limited to the last 5 years. We included observational studies from South American countries published in Spanish, English and Portuguese; those that included the proportion of awareness, treatment and control of participants with diabetes. A qualitative synthesis was performed with the main results, and a meta-analysis of each outcome using the random effects model. Additionally, the risk of bias was assessed. Results: 14 studies met the inclusion criteria. The disease awareness meta-analysis included 5 studies and found an estimate of 72,2% (IC95%: 64,0%-80,0%, I2: 93,1%). The disease treatment meta-analysis included 3 studies and found an estimate of 66,3% (IC95%: 52,6%-78,8%, I2:98,6%). The disease control meta-analysis included 5 studies and found an estimate of 43,8% (IC95%: 38,6%-49,0%, I2:95,0%). All of our studies were at low risk of bias. Conclusions: In South America, the proportion of people with diabetes that are not aware of their disease, not treated and not controlled are high. Measures to improve diabetes diagnosis, treatment and control should be implemented.
