Background: The increase in the mortality was seen worldwide with the beginning of the COVID-19 pandemic. In Latin America, an increase in maternal deaths was reported. Objective: To compare the characteristics of maternal mortality in Peru before and during the COVID-19 pandemic in the period between March 2017 to February 2021. Methods and materials: We made a cross-sectional study from the SINADEF database. A sociodemographic description of the maternal deaths were made, and the maternal mortality ratio and the maternal mortality rate were calculated, nationally and by departments. In addition, maternal deaths from COVID-19 were described. Results: We found an increase in the frequency of maternal deaths from the prepandemic period to the period during the pandemic. The maternal mortality ratio in the prepandemic was 26.9 maternal deaths per 100 000 live births, while during the pandemic, 52.9 maternal deaths per 100 000 live births. Apurimac, Tumbes, Tacna, and Pasco were the most affected deparments. Hypertensive disorders during pregnancy, childbirth, and puerperium, and no obstetric causes, mostly due to COVID-19, predominated during the pandemic. A gap between the SINADEF database and the CDC reports were found. Conclusions: The impact of the pandemic was evidence in the increase of the maternal deaths, the maternal mortality ratio and the maternal mortality rate, nationally and by departments. It is necessary the collaboration between SINADEF and NotiWeb to improve the situation of maternal mortality in Peru.
