Background: Tuberculosis, an infectious disease with high mortality, is classified into pulmonary and extrapulmonary tuberculosis, the elderly are more susceptible to the latter due to their lower physiological immunity, being a diagnostic challenge. In the review, associated factors were found, but are controversial in our population since there is no analytical study. Objective: To identify the risk factors for extrapulmonary tuberculosis (EPTB) in the elderly at Cayetano Heredia Hospital (CHH) between 2014- 2021. Materials and methods: Analytical observational study of cases and controls, retrospective based on medical records of the elderly. The variables to be analyzed were 6, 5 of them categorical: CKD, DM, neoplasms, sex and history of pulmonary TB, and one numerical: age. Univariate analysis with summary measures, bivariate t-student analysis for age and for the remainder chi squared with 95% CI and p < 0.05 as statistically significant. Finally, multivariate analysis with logistic regression for the degree of association with odds ratio (OR). Results: History of pulmonary tuberculosis has 21.5 times more risk (OR= 21.509) for EPTB. The oncological diagnosis has 18 times more risk for EPTB (OR = 18.051). During the investigation, it was identified that the TB contact (OR=153) has 0.15 times less risk for EPTB. Conclusions: A history of PTB and cancer diagnosis are risk factors, but history of TB contact is a protective factor for EPTB.
