SARS-CoV-2 is the virus that is responsible for the current COVID-19 pandemic, it induces an inflammatory and adaptive immune response that leads to a variation in the proinflammatory cytokine expression. Therefore, there is an imbalance in the proliferation of immune cells, such as lymphocytes and neutrophils, as well as platelet production at a systemic level. There are precedents of the use of biomarkers like the neutrophil to lymphocyte ratio (NLR, platelet to lymphocyte ratio (PLR) and medium platelet volume to predict and diagnose the outcome of different acute and chronic inflammatory pathologies, because of their accessibility and low cost. For these reasons this study has the objective to identify the predictive value of NLR, PLR and VPM over the mortality of a COVID-19 patient cohort that were admitted to Cayetano Heredia National Hospital (HCH) in Lima Peru. The present investigation is a primary analysis of a data base from a prospective cohort study (SIDISI: 204190). This principal study analyzed a cohort of 100 COVID-19 patients that were confirmed by a serological or molecular COVID -19 test and were interned at the HCH during the months of October 2020 and January 2021. We used the neutrophil, lymphocyte, and platelet absolute count from the automated hemogram to calculate NLR, PLR and VPM values. We present T-Student tests as well as Areas under the curve (AUC) with a 95% confidence interval to determine the predictive value of the biomarkers of interest. Our results show statistically significant differences (p<0.05) in neutrophils, lymphocytes, red blood cell distribution width (RDW), platelets and NLR between the deceased and survivor groups. We observed an increase in the neutrophil concentration, RDW and NLR values in the deceased group versus the control group. There was also a reduction in the absolute lymphocyte and platelet count in the deceased COVID-19 group. There were no significant differences in the other biomarker values, PLR and VPM, between groups, p=0.053 and p=0.07 respectively. The NLR biomarker obtained the best AUC value in the model adjusted by sex, age, and number of comorbidities, with 8 a value of 0.945 [0.88-1.00], specificity of 76.82% and sensitivity of 100%. The cutoff value for the classification of NLR was 8.91. With these results we conclude that NLR has a predictive value over the mortality of COVID-19 patients.
