BACKGROUND: Weight gain of infants less than 1500 grams at birth, during hospitalization is a problem, despite the use of breast milk with different commercial fortifier not know the results of fortified low-cost home. OBJECTIVES: To compare weight gain, height and head circumference and change in the z-score with two schemes of fortification of breast milk. Evaluate changes in hemoglobin, albumin, calcium, phosphorus, alkaline phosphatase, and hospital stay for each feeding scheme. STUDY DESIGN: A randomized, controlled double-blind. PERSONS AND METHODS: We studied 44 newborns weighing less than 1500 grams, still with full enteral nutrition. The first group (n = 22) received breast milk fortified Scheme 1 (Home) and the second group with same (n = 22) received breast milk fortified Scheme 2 (Commercial). RESULTS: Weight gain (g / day), height (cm / week) and head circumference (cm / week) was similar to Scheme 1 and 2 (24.54±4.45 vs 25.64±5.62, 0.59±0.32 vs. 0.54±0.32; 0.62±0.25 vs. 0.75±0.21 respectively, (p> 0.05). The z-score at discharge in both groups were: weight 1.86 ± 0.71 vs -1.68 ± 1.0, height -1.53 ± 0.19 vs -1.26 ± 0.24 and head circumference - 2.17 ± - 2.2 vs 0.94 ± 1.12 respectively. The hospital stay was similar in both groups (p> 0.05). At the end of the study, plasma levels of albumin and phosphorus were lower compared with Scheme 1 with Scheme 2 (3.26±0.11 vs 3.53±0.08 and 4.81±1.12 vs 6.25±1.07mg/dl respectively);p<0.05) and the delta calcium (-0.58±1.00 and 0.64±1.28;p<0.05). CONCLUSIONS: The two schemes showed similar results. With the exception of plasma levels of albumin, calcium and phosphorus were lower with Scheme 1 but with normal range without showing osteopenia of prematurity.
