The industrial fisher population works for long periods on vessels and facing sedentary lifestyle conditions, dietary changes, stress and unhealthy habits. Being exposed to these factors permanently would increase the risk of metabolic alterations and consequently, degenerative chronic diseases such as type 2 diabetes mellitus (DM2). Insulin resistance is a hallmark of metabolic syndrome and is the main predictor for DM2 and other cardiovascular diseases. Various studies apply indirect estimated for its calculation, including the TYG index that leverages easy to obtain data on glucose and tryglicerides in blood samples, increasing its practicity compared to other methods. The present study is a retrospective analysis medical occupational reports of Peruvian industrial fishermen. The objective is to estimate the risk of presenting IR in an open cohort of fishermen that work exclusively in vessels compared to those not working at the sea. Incidence rates, relative risk and crude and adjusted Cox regression models are presented. A total of 643 observations were collected from Peruvian industrial fishing workers, 203 in the unexposed and 440 in the exposed group, within 4 years of follow-up. Under the selection criteria, a cohort of 97 participants was formed (67 exposed and 30 unexposed). A RR of 1.58 (95% CI 0.8 - 3.4) was estimated. In the Cox regression analysis, a HR of 1.27 (95% CI 0.7 - 2.9) is presented for the crude model and of 1.16 (95% CI 0.6 - 2.4) for the model adjusted for hypercholesterolemia and BMI. We conclude that there is not enough evidence to associate the risk of IR with the activity of industrial fishing vessels. However, more adverse metabolic results are observed in the exposed group. More studies are required with a greater quantity and time of follow-up in the industrial fishing population.
