Abstract: Introducción: En pacientes con EHGNA, la “Diabetes mellitus tipo 2” es considerada el principal factor de riesgo de fibrosis hepática. Considerando que no siempre es posible realizar la biopsia hepática, nosotros pretendemos con este trabajo identificar los factores clínicos, bioquímicos y antropométricos relacionados con fibrosis hepática avanzada, diagnosticados por biopsia hepática, en pacientes obesos con DM2. Objetivos: Identificar los factores relacionados con fibrosis hepática avanzada, diagnosticados por biopsia hepática en pacientes obesos con diabetes mellitus tipo 2. Tipo de estudio: Descriptivo, retrospectivo, serie de casos. Material y métodos: Se revisó historias clínicas de los pacientes obesos con DM2 mayores de 18 años sometidos a cirugía bariátrica en una clínica privada en Perú entre enero 2019 a enero 2021. Se estratificó en 3 grupos con hígado normal, esteatohepatitis con fibrosis temprana y avanzada. Resultados: Cuarenta y un pacientes obesos tuvieron DM2. 56% fueron varones. 6 tuvieron hígado normal y 35 esteatohepatitis, de los cuales 12 presentaron esteatohepatitis con fibrosis avanzada. La edad estuvo relacionada significativamente a la presencia de fibrosis temprana y avanzada (p=0.006). El valor de HOMA-IR estuvo relacionado a la presencia de EHNA con fibrosis con un valor de p de 0.033. También, estuvieron relacionadas significativamente la GGT (p=0.011) y la TGO (p=0.009). Conclusión: La edad, el índice de resistencia a la insulina, la GGT y la TGO fueron los factores relacionados con la presencia de fibrosis hepática temprana y avanzada en nuestros pacientes.
Introduction: In patients with NAFLD, "Type 2 diabetes mellitus" is considered the main risk factor for liver fibrosis. Considering that it is not always possible to perform a liver biopsy, we intend with this work to identify the clinical, biochemical, and anthropometric factors related to advanced liver fibrosis, diagnosed by liver biopsy, in obese patients with T2DM. Aim: To identify the factors related to advance liver fibrosis, diagnosed by liver biopsy in obese patients with type 2 diabetes mellitus. Study type: Descriptive, retrospective, case series. Material and methods: Clinical records of obese patients with T2DM over 18 years of age who underwent bariatric surgery in a private clinic in Perú between January 2019 to January 2021 were reviewed. The population was stratified into 3 groups with normal liver, early fibrosis, and advanced fibrosis. Results: Forty-one obese patients had T2DM. 56% were male. 6 had a normal liver, and 35 steatohepatitis, of which 12 had steatohepatitis with advanced fibrosis. Age was significantly related with the presence of early and advanced fibrosis (p = 0.006). The HOMA-IR was related with NAFLD with fibrosis with a p-value of 0.033.
