Introduction: Sexual role (insertive, receptive y versatile) has been recognized as a risk factor for Sexually Transmitted Infections (STI) among men sex with men (MSM). Infection persistence due to high risk human papillomavirus (HPV) genotypes is associated to anogenital cancer between men. Sexual role structure in MSM and behavioral factors determine HPV transmission and cancer. This study aimed to evaluate sexual role and the number of high risk HPV genotypes infecting anal and genital tissues among MSM. Methods: This study analyzed data from a cross-sectional study, where MSM from Lima were recruited through respondent driven sampling. Sexual role, the exposure variable, was collected with an electronic survey. The outcome, number of high risk HPV genotypes in anal and genital infections, was determined using a PCR and a 39-genotype hybridization test in anal and genital swabs. Central statistical analysis was performed using a zero-inflated negative binomial regression using mean ratios (MR) as estimators. Results: Two hundred participants between 18-59 years old (mean 34.1 ± SD 9.4) were recruited and analyzed. We found a 71.5% prevalence for high-risk anal HPV, and a 37.2% prevalence for high-risk genital HPV. Fifty percent were primarily receptive, 35% primarily insertive and 14.5% versatile. Being versatile was associated to higher number of high-risk genital HPV (aMR: 1.62, 95%IC: 1.00-2.61, p=0.048). Association between sexual role and number of high-risk anal HPV was not found. Moreover, participants HIV positive were at higher risk for developing infections with more high-risk anal HPV types. Conclusions: Versatile role represents a more vulnerable group to high-risk genital HPV. Furthermore, HIV remains a risk factor for high-risk anal HPV. Interventions, such as vaccination, to prevent HPV infections, or interventions that promote early diagnosis, should be prioritized among these groups.
