Introduction: The present research work arises from a concern regarding the lack of consensus on the concept of posterior bite collapse, a controversial term on which there is currently very little literature. The present systematic review would clarify the clinical characteristics of this pathology. Objective: To identify the relevant clinical factors in the diagnosis of a posterior bite collapse. Materials and methods: The present research is a systematic review based on reports and case series in PubMed, EMBASE, Scopus, Cochrane and the Virtual Health Library (VHL) until August 12, 2020. Dental tissue was considered as dimensions of the clinical characteristics, dental occlusion, muscle tissue, temporomandibular joint, periodontal tissue and radiographic examination. Compliance with the writing of the case reports was evaluated following the recommendations of the Case Report (CARE) guide and then the risk of bias following the methodology proposed by Pierson. Results: The review was carried out with 6 articles, 6 (100.00%) of them fulfilled most of the CARE criteria; According to the risk assessment, 3 (50.00%) presented moderate risk and 3 (50.00%) were low risk. With the articles, the information was ordered describing each one of them, where 1 of them was a case series of 3 patients, the articles were from Korea, the United States, Indonesia, Israel and Japan; The patients were mainly female, with an age range between 20 to 64 years, with publications between 1987 and 2020. Conclusions: Based on the systematic review of the case reports analyzed, the relevant clinical factors for the correct diagnosis of a posterior bite collapse are identified according to the study dimensions: loss of teeth, alteration of the vertical dimension, periodontitis, deviation in the opening and closing of the TMJ and bone resorption, as the most predominant according to the diagnostic dimensions.
