Background: The evolution of gastric dysplasia towards gastric cancer occurs in less than one year. To prevent this, it is crucial to histochemically classify intestinal metaplasia. However, few investigations have evaluated the association of the histochemical type of intestinal metaplasia with gastric dysplasia. Objective: To evaluate whether there is an association between the histochemical type of intestinal metaplasia and the presence of gastric dysplasia in the population of histopathological diagnoses of gastric biopsies with intestinal metaplasia. Methods: Secondary analysis of data from a cross-sectional study. The association of the histochemical type of intestinal metaplasia and the presence of gastric dysplasia was mainly evaluated using a multiple regression model with an epidemiological focus. Results: The prevalence of gastric dysplasia in cases with intestinal metaplasia of columnar changes type is 8.16 times the prevalence of gastric dysplasia compared to those without columnar changes, adjusting by age, presence of atrophy and presence of Helicobacter pylori. This result is statistically significant (p <0.001) with a 95% CI: 4.57 - 14.56. Conclusions: The histochemical type of intestinal metaplasia with columnar changes is associated with the presence of gastric dysplasia.
