ABSTRACT The main objective of this research was to determine the effect of pharmacotherapeutic follow-up on drug-related problems in diabetic patients in a level III hospital in Trujillo- Peru. The methodology of the study is retrospective descriptive and for the intervention the SOAP method was used following the BPOF procedure. 21 patients between 35 and 90 years of age were included for a period of 6 months. It was determined that the most prevalent population with DM2 was female with 61.9% and the most prevalent age group was 71 to 75 years with 23.8%, in addition, pharmacotherapeutic follow-up allowed control of clinical parameters such as fasting glucose (76.2%), postprandial glucose (81%), HbA1c% (76.2%), blood pressure (85.7%), LDL cholesterol (66.7%), triglycerides (61.9%), except for IMC (33.3%); The most prevalent PRM were effectiveness (41.1%), safety (35.6%), and finally, necessity (23.3%), of which 84.9% of PRM were resolved, with the development of 70 pharmaceutical interventions, which were by drug quantity (38.6%) and pharmacological strategy (61.4%), of which 65 (92.9%) interventions were accepted, 95.4% of which resolved PRM and 4.6% were not resolved, and 5 (7.1%) interventions were rejected. Finally, it was concluded that pharmacotherapeutic follow-up through the evaluation of the need, effectiveness and safety of pharmacological treatment allowed the detection and favorable resolution of DRPs in diabetics, making it possible to optimize their pharmacotherapy
