Treatment planning systems (TPS) are used to determine the absorbed dose (Dabs) received by the tissues of cáncer patients on brachytherapy: but some of these systems do not consider the heterogeneities of the tissues, the types of applicators and the size of the patient, which generates diíferences between the Dabs calculated by the TPS and the Dabs received by the patient, Thus, the question arises whether the TPS used adequately calculates the distribution of Dabs in the patient and what the effects are of not considering the heterogeneities of the médium, The present study proposes a method to test the calculation model of the Dabs of the TPS, in order to determine its accuracy. This method consists of three steps: • Proof of library, We compute the distribution of Dabs, obtained by the TPS and theoretically, The theoretical calculation of the Dabs distribution around an 192 Ir source in water, is performed using the formalism of the AAPM TG-43, • Isodosis calculation test, for two cases: i) The case of a stoppage schedule of the 192Ir, taking the heterogeneities of the médium into consideration, ii) The case of a schedule with three stoppages of the 192Ir source, considering the attenuations of the tissue and the applicators, These calculations are performed using Monte Cario simulation (MC) with Gamos codes of 5,1,0, In the first case, the Dabs around an 192Ir source in water is simulated in order to validate the simulation, Further simulations are then performed to quantify the attenuation of the Dabs when the attenuation of the tissue and of the applicators is considered, In the second case, we proceed to compute the distribution of the Dabs in a three stop arrangement of the source, considering the attenuation effect of human tissue heterogeneities and applicators, • Dose accuracy test delivered, Using a PPMA phantom, which simulates human tissue, in which the brachytherapy applicators and the 192Ir source are inserted, using the TPS a three stop treatment of 192Ir is planned, as in the second case of the isodosis calculation test, in order to deliver a Dabs to OSL nano dot dosimeters, Finally, we compute the Dabs planned with that registered by the dosimeter, This method was applied to the ARIA RADonc TPS and it was discovered that the difference between the theoretical calculation of the Dabs distribution around the 192Ir source immersed in water and calculated by the TPS is in a margin of 1 %, meeting the tolerance level of 2% established by ESTRO, In the isodosis calculation test around the 192Ir source, when immersed in water, a similarity rate of 97% of the gamma Índex was obtained between that calculated with the TPS and MC, using as gamma Índex tolerance the valúes of Ad = 3 mm and AD = 5 %, In the isodose calculation test in the presence of heterogeneities with MC, it was found that indeed there are? significant differences, ranging from 0 % to 30 %, depending on the geometric configuration between the source and the applicators, as well as the gamma Índex that indicates a similarity of plans of 60% in the case of a plan with three stoppages of the source: such tests were performed with a voxel síze of 8 mm, In the treatment Dabs accuracy test, with the OSL, a sub-dosage of 3,88% is reported and the precisión in the treatment delivery has a variation coefficient of 1,4%, To conclude, the TPS adequately computes Dabs, when the médium is water: but it is necessary to inelude the libraries that consider the heterogeneities, to make a more accurate calculation of the Dabs distribution in patients.
