The re-admission rate is due to multiple factors, whether clinical, hospital-derived and the patient's own. The objective is to apply pharmaceutical care to patients with high internal medicine of the HDCQ "Daniel Alcides Carrión" -Huancayo, using a study as a methodology of the applied level of explanatory type and quasi-experimental design, carried out in a sample of two groups, one experiment and another control group, obtaining as results that the therapeutic drugs prescribed at hospital discharge were Losartan in 38% (11), Metformin 28% (8 ), Acetylcysteine 17% (5) in the experimental group as far as the control group are located Losartan 34% (10), Insulin 28% (8), Acetylcysteine 17% (5); The experimental group met 88 (IF), concluding that pharmaceutical care in 97% (28) of the cases studied, did not reenter the hospital in the experimental group, while in the control group they re-entered in 31%, of which they did not receive information from the pharmacist.
