The present research study was carried out in the archives of the Clinical Histories of the Stomatology Clinic of the Professional School of Stomatology of the Universidad Privada de Huancayo Franklin Roosevelt in the year 2019. Due to the high rate of surgical difficulty that mandibular third molars present, due to their lack of space, eruption time and the multiple pathologies that this dental piece generates. The main objective was to establish the predominance of the degree of surgical difficulty of mandibular third molars according to the Koerner Index in panoramic radiographs in our region. As for the methodology used for the study sequence period, it is retrospective transversal and according to its descriptive level. The sampling was non-probabilistic using the method of convenience, conformed by 58 panoramic radiographs from 18 to 40 years old. The results were found from a total of 116 lower molars. Moderate difficulty predominated with 56.0% (n=65). This was followed by minimal difficulty with 31.9% (n=37) and finally very difficult with 12.1% (n=14)
