The present research work is based on an innovation project to improve the availability of Clinic Records and thus reduce the waiting time of external users for care in outpatient at the “Regional Teaching Hospital of Cajamarca”. Therefore, the problem and its causes were determined such asthe deficit of equipment issuing tickets, poorly trained personnel, attention by order of arrival of external users, Clinical History not available for attention for care outpatient consultation, and the absence of the doctor in the outpatient clinic. This work is part of applied research and seeks to generate innovation in the face of a problem. A hierarchy of the causes was made based on criteria referring to the level of impact on the problem found, modification and if it was found in the normative scope of the Organization from which it intends to generate the intervention. The problem was reformulated and, in this way, the innovation challenge became: How can we improve the availability of the Clinical History of external users to reduce the waiting time for care in Outpatient Department of the Regional Teaching Hospital of Cajamarca? The innovation prototype is made up of a central axis that is the “Computer application for monitoring Clinical Histories“, and four components such as training on the management of Clinical Histories, the physical rearrangement of Clinical Histories, Administrative Directive of Management Processes of Clinical Histories, and health training aimed at improve the subjective waiting time of external users in Outpatient Consultation. Finally, after testing the prototype in the hospital, in addition to complying with the analyzes of desirability, feasibility and viability, it is concluded that the prototype will generate public value, social relevance, practical usefulness and above all a greater benefit to citizens.
