Objective: To determine the relationship between the platelet-lymphocyte ratio (PLR) and mortality in patients with severe dengue at EsSalud José Cayetano Heredia Hospital in Piura during the years 2022-2023. Methods: A retrospective cohort study was conducted, focusing on hospitalized patients with severe dengue. The sample included 30 patients, with 10 deceased cases and 20 surviving cases. Inclusion criteria were adults diagnosed with severe dengue and complete medical records. Mortality was considered the dependent variable, and PLR the independent variable. Results: A total of 30 patients were evaluated with a mean age of 52.1 years. The sex distribution was slightly higher in men (53.3%). The most common comorbidities were arterial hypertension (30%) and type 2 diabetes mellitus (16.7%). Deceased patients had significantly lower platelet levels (23.5 x10³ cells/mm³) and lymphocyte levels (1.1 x10³ cells/mm³) compared to survivors. The prevalence of mortality was higher in patients with a low PLR (53.3%) compared to those with a normal PLR (13.3%), this difference was statistically significant. Conclusions: The association between a low platelet/lymphocyte ratio and mortality in patients with severe dengue was statistically significant. This suggests that there is a trend toward higher mortality in patients with a low IPL. Further studies with larger sample sizes are recommended to confirm these findings and to consider other clinical factors and comorbidities in assessing the risk of mortality in patients with severe dengue
