Objective: To evaluate the association between the rational use of antibiotics and hospital outcomes in patients with severe pneumonia treated in the emergency services of EsSalud José Cayetano Heredia III Hospital in Piura. Materials and Methods: This cross-sectional study was conducted at EsSalud José Cayetano Heredia III Hospital in Piura from June 2021 to June 2023. The study population included all patients with severe pneumonia treated in the hospital's emergency services during this period. The sample comprised patients selected based on specific inclusion and exclusion criteria. Results: The study at EsSalud José Cayetano Heredia III Hospital in Piura examined patients with severe pneumonia, finding a mean age of 40.23 years and a male predominance (64.56%). 82.52% received appropriate antibiotic use. Of the patients, 14.08% died, 39.3% required prolonged hospitalization, and 48.1% were discharged. A significant association was found between the rational use of antibiotics, age, immune status, and hospital outcomes. Conclusions: This study highlights the importance of the rational use of antibiotics in patients with severe pneumonia, significantly associated with hospital outcomes. It also identifies useful clinical patterns, confirms the prognostic relevance of age, and emphasizes the crucial role of immune status in hospital outcomes for these patients
