To demonstrate the association between the Prognostic Nutritional Index (PNI) and intra-abdominal complications in surgery for lower rectal cancer. Materials and method: An observational case-control study was carried out nested in a cohort that considered a census population, which included 14 patients with intra-abdominal complications (cases) and 20 without intra- abdominal complications (controls), during the period between the years 2007 and 2023 at the Regional Institute of Neoplastic Diseases (IREN) of Trujillo, Peru. Results: Of the group of cases, it was found that 26.5% presented INP ≤ 43, compared to 14.7% who presented INP > 43. No significant association was found between an NPI ≤ 43 and the development of intra-abdominal complications (p=0.092; OR:3.34; IC: 0.80– 13.94). Other variables such as age, BMI, sex, clinical stage, histological type and blood transfusion did not show a significant association with intra-abdominal complications (p=NS). Conclusions: The Prognostic Nutritional Index (PNI) has no association with intra-abdominal complications in patients who had surgery for lower rectal cancer treated at the IREN Trujillo, Peru.
