-- Aim: To evaluate the benefits of implementing a monitoring form for patients with kidney disease at the Renal Health Program of the Healthcare Network Almenara- EsSalud, during 6 months, compared to 6 previous months before the intervention. Methods: Using a quasi-experimental design we evaluated the Renal Health Program at the Healthcare Network Almenara- EsSalud, 6 months previous and 6 months during the implementation of a follow-up form to monitor the patients with kidney disease. During this evaluation the following parameters were compared pre and post the intervention: total of new patients, percentage of new patients with early chronic kidney disease ([CKD] grades I, II or III / total new patients with CKD), ratio of references/counter-referrals, and rate of patients in follow-up (Total number of patients on follow-up/Total patients at first consultation) Results: During the period January-December 2009 a total of 206 new patients were treated with a diagnosis of chronic kidney disease, 94% having early disease. After analyzing the potential benefits of the implementation of a follow-up form were found both a significant increase as a linear trend in each of the following indicators: monthly amount of new patients (12.7 ± 5.2 vs. 21.5 ± 3.7, p=0.007, 7.79 ± 1.43 * month, R2=0.6724, p = 0,001), monthly proportion of new patients with early CKD (90.5 ± 4.8% vs. 94.5 ± 1.6%, p=0.002, R2=0.3422, p=0.046), the average referrals (288.7 ± 17.9 vs. 355.0 ± 63.0, p=0.033, R2=0.231, p=0.114) and counter-referrals cases per month (181.2 ± 38.1 vs. 296.7 ± 69.4, p=0.005, R2=0.475, p=0.013), the ratio of counter-referrals and referrals (0.63 ± 0.15 vs. 0.83 ± 0.09, p=0.020, R2=0.4731, p=0.013) and the monthly percentage of patients counter-referred on follow-up (49.3 ± 1.9% vs. 59.6 ± 4.1%, p<0.001, R2=0.8086, p<0.001). Conclusions: The instrumentalization of the Renal Health Program by implementing a follow-up form had a positive impact on the effectiveness of the program significantly improving the monthly amount of new patients, the monthly proportion of new patients with early CKD, the average referrals and counter-referrals cases per month, the ratio of counter-referrals and referrals and the monthly percentage of patients counter-referred on follow-up. -- Keywords: Kidney disease, Kidney Health, EsSalud, Monitoring, Instrumentation
