--- The choice of antibiotic to treat Community-Acquired Pneumonia (CAP) is empiric and based on factors that influence on determined types of germs. The objective of this study was to determine the effectiveness of the empiric antibiticoterapia in patient with CAP that follow the guides ATS (American Thoracic Society) and to compare them with other antibiotic outlines that are not recommended. METHOD In this prospective study, cohort of observational type 70 patients was included, 35 per group, from both sex with CAP that required hospitalization. The patients were separated in those that followed the guides ATS (cases) and those that didn't make it(control). In the beginnig they registered the clinical, biochemical and radiological characteristics in each case, and the 96 hours the effect was evaluated, being considered effective <0 and ineffective >= 0. For the analysis was used parametric tests, squared Chi, Odds ratio and tests of logistical regression. RESULTS The age > to 65 years and the multiple comorbidity was the risk factors predominant in both groups. The patients that received treatment according to ATS in the radiological, clinical and biochemical evaluation to the 4to day were better (with effectiveness of -3.54) in relation to those that didn't follow the ATS guides (with effectiveness +1.71). Comparing both groups an OR=362.67 (30.76 <OR <10491) and RR 11.3 (3.84 <RR <33.49 was found. CONCLUSIONS The antibiotic empiric according to ATS is effective in the CAP treatment.
