Abstract: El hombre es el hospedero definitivo de la Taenia solium, céstodo zoonótico, y la única fuente de infección de la cisticercosis para los cerdos y los humanos.
Las especies más comunes que afectan al hombre son Taenia solium y Taenia saginata, cuya distribución es cosmopolita. La detección de portadores humanos del gusano adulto constituye uno de los pilares fundamentales para la culminación del ciclo biológico. Las técnicas convencionales para el diagnóstico de portadores de Taenia sp se basan en el hallazgo de huevos en heces, el ELISA de captura (coproantígeno), y el diagnóstico serológico. El objetivo del trabajo fue evaluar los métodos diagnósticos para la teniasis intestinal en zonas endémicas del Perú. El estudio se llevó a cabo durante los años 1997 al 2009.
La población de estudio consistió en 324 pacientes, que acudieron al IECN por sintomatología neurológica además de pacientes provenientes de estudios de campo, que eliminaron todos ellos Taenia sp, después de recibir tratamiento.
Resultados: De los 324 pacientes, 299 eliminaron T. solium y 25 eliminaron T.
saginata. De los pacientes que eliminaron T. solium, 60% (178/299) fueron de sexo femenino y 40% (121/299) fueron de sexo masculino. El 86% (259/299) de nuestros casos fueron diagnosticados por microscopia, y 95% (144/151) por coproantígeno. Así mismo, se obtuvo una muestra de suero por paciente la cual fue evaluada por Western blot con antígeno recombinante rES33, donde 99% (164/166) fueron positivos. De los pacientes que eliminaron T. saginata, el 100% (25/25) de los casos fueron positivos a microscopia, ninguno fue diagnosticado por coproantígeno. Así mismo ningún suero fue positivo al recombinante rES33 (0/20). El riesgo de adquirir neurocisticercosis (NCC) fue evaluado en 120 pacientes con teniasis confirmada por T. solium.
Conclusiones: Se puede observar que hay una tendencia del sexo femenino a adquirir teniasis. El diagnóstico por coproantígeno ha demostrado detectar solamente casos de teniasis por T. solium, lo mismo que el recombinante rES33; probablemente la combinación de ambas pruebas mejore aun más la posibilidad de detección. Se encontró que el riesgo de adquirir NCC fue bajo en áreas no endémicas y que la NCC (cisticercos viables) fue encontrada sólo en pacientes sintomáticos y familiares.

Detection of human carriers of adult worms is one of the pillars for the breaking of the cycle. Conventional techniques for the diagnosis of carriers of Taenia sp. are based on finding eggs in feces, capture ELISA (coproantigen) and serological diagnosis. The objective was to evaluate diagnostic methods for intestinal taeniasis in endemic areas of Peru. The study was conducted during 1997 to 2009. The study population consisted of 324 patients who attended the IECN by neurological symptoms in addition to patients from field studies, which eliminated all Taenia sp. after treatment. Results: Of 324 patients, 299 eliminated T. solium and 25 eliminated T. saginata. Of the patients who eliminated T. solium, 60% (178/299) were female and 40% (121/299) were male. 86% (259/299) of our cases were diagnosed by microscopy, and 95% (144/151) for coproantigen.
