--- A descriptive, retrospective and cross-sectional study was performed to determine the pre and intra-operative factors associated with postoperative complications in mandibular fractures in patients treated at Dos de Mayo National Hospital in Lima during a 5-year period (from 2009 to 2013). A total of 111 mandibular fracture cases were found. 58 patients were the sample. Malocclusion, infections, maxillofacial neurological disorders and changes in the consolidation process among the postoperative complications were evaluated. The evaluated preoperative factors associated with them were the following: the waiting time until the surgical procedure, the etiology of the fracture, patient cooperation, systemic disease and the type of fracture. The evaluated intra-operative factors were type of treatment and the type of approach. The results were analyzed using descriptive statistics and the Fisher exact test. It was concluded that the patient cooperation factor had association with postoperative malocclusion where the 43.8% of patients with this complication presented unfavorable attitude. Systemic disease and the type of fracture determined by soft tissue injury also showed association with postoperative infectious processes, where out of the total number of cases with this complication, 40% had hypertension and 100% were open fractures. The type of fracture determined by fracture line direction showed association with the types of neurological maxillofacial disorders, where 44.4% of all cases with neurological impairment had hypoesthesia in fractures with unfavorable line direction. No preoperative or intraoperative factor presented association with changes in the consolidation process. Keywords: Mandibular fracture – postoperative complication – preoperative factor - intraoperative factors.
