--- Objectives: The present study aimed to know if the immediate induction of labor with oxytocin in patients with PROM at term is better than waiting its spontaneous beginning within the following twelve hours of the rupture. Material and methods: We studied 363 patients with PROM at term that were admitted at HONADOMANI SAN BARTOLOME between May 2004 and February 2005. They were randomly assigned, 116 to the expectant management group for 12 hours and 115 to the group of labor induction. The primary end-points were: maternal-neonatal rate of infection, rate of vaginal birth and the duration of the latency period in both groups. Results: The average age was 25,63 years for the induction group and 25,21 for the expectant management group (p =). 70,4% of women in the induction group were nulliparas and 65,5% in the other group (p =). None of the newborns presented infection. Only 3 women (2.6%) from the expectant management group presented maternal infection. There was need for a cesarean section only in 4 patients of the induction group due to failure of induction. The latency period in the induction group was 2,91 hours compared with the average 7,34 hours of the expectant management group. Conclusion: The active management of PROM at term with oxytocin is as effective as its expectant management for 12 hours protocolized in our hospital, based on what we propose that this conduct should be taken as an effective option when deciding the management of PROM at term.
