--- This study is observational and prospective, which main objective is to determinate the economic impact of the Pharmacotherapy follow-up performed by the Clinical Pharmacist who participates actively involved in medical visit, to analyzing his contribution to reducing health care costs in hospitalized patients in the Medicine Division of the National Hospital of the National Police of Peru “Luis N. Sáenz“ for a period of six months, for which a bachelor in Pharmacy and Biochemistry integrated the health team in different Medicine Division´s services. The Pharmacotherapy follow-up methodology was applied selecting patients in which were identified and classified the drug-related problems (DRPs) real and potential. The severity of the detected or resolved PRM and risk of increasing of hospital stay were correlated and cost potentially avoided was estimated. 487 drug-related problems were identified in 152 patients, of whom 74.7% (364) were potential, the estimating avoided cost were S/.93362.42 ($ .29 648.28). It was observed that the avoided cost per patient (S/. 911.08) was higher in the group over 65 years; it was determined for each patient, who received treatment with the group of antineoplastic drugs, showed a higher avoided cost (S/. 425.3) (p <0.05) than the others. In conclusion, the integration of the pharmaceutics in the health team allows to identifies potential problems related to drugs and prevent them, reducing the risk of increased hospital stay and health care costs.
