Objective To evaluate the psychometric properties of the Kessler K6 Scale for severe mental illness in adults in California, USA in 2019. Methods We carried out an instrumental study using the 2019 California Health Interview Survey (CHIS) data. The Kessler K6 Scale is a screening tool that identifies individuals at risk of severe mental illness through a six-question questionnaire. We performed an exploratory factor analysis (EFA) to assess the scale's internal structure, followed by a confirmatory factor analysis (CFA) using the robust maximum likelihood estimation method where we applied the Tucker-Lewis Index (TLI) and the Comparative Fit Index (CFI). We also examined the correlation between K6 and loneliness symptoms (Three-Item Loneliness). Finally, measurement invariance analyses were carried out, and reliability was assessed using McDonald’s omega coefficient. Results A total of 22,048 individuals were included in the analysis, and EFA identified the unidimensional model for the K6. CFA showed optimal values with CFI and TLI of 0.995 and 0.992, respectively, and we found a relationship between items AJ29 and AJ31. Regarding the correlation with other variables, the K6 has a weak negative association with the Three-Item Loneliness Scale. Measurement invariance was found across age, gender, disability, and annual income groups. Finally, the results revealed high internal consistency with a classical alpha coefficient of 0.873 and a categorical omega coefficient of 0.932 and 0.926. Conclusions The Kessler K6 scale showed suitable psychometric properties for severe mental illness in California adults and achieved measurement invariance across groups divided by gender, age, annual income, or disability.
