TITLE: “Association between illicit drug use and poor adherence to highly active antiretroviral therapy in adults with HIV-AIDS infection in Peru: systematic review and meta-analysis.” OBJECTIVES: Determine the existing evidence of the association between illicit drug use and poor adherence to HAART in patients with HIV-AIDS infection in Peru. METHODS: The inclusion criteria were the following: scientific articles with a cross-sectional design, cohort, case control in indexed scientific publications and gray literature in an online version with the Peruvian population, in Spanish, English and Portuguese in full text, which were carried out in Peru. Furthermore, the range of year of publication for the selection of studies was from the beginning of each database onwards, studies that consider as a population adults over 18 years of age with a confirmed diagnosis of HIV-AIDS infection on HAART and studies that describes the association between illicit drug use and poor adherence to treatment in patients with HIV-AIDS infection. In addition, a search of available literature was carried out in databases such as PubMed, SciELO, Scopus, Web of Science, Embase, Google Scholar; LILACS and ALICIA for the search for undergraduate and postgraduate theses (search carried out until April 7, 2023), using structured search terms. The risk of bias was assessed using the Newcastle Ottawa scale, finding a high risk of bias in the cross-sectional and case-control studies. In addition, a meta-analysis was performed using the REVIEW MANAGER 5.4 program. RESULTS: 6 articles were included, with a total of 1158 participants. Within the studies, 50% were cross-sectional in design; the rest were cases and controls. Furthermore, the articles included for analysis were thesis type carried out in Peru. A frequency of drug consumption in patients with poor adherence to HAART was found to be 2.4 - 65.8%. A meta-analysis was performed; With subgroup analysis by study design with case-control studies, a statistically significant association was found between illicit drug use and non-adherence to HAART (OR=4.74; 95% CI=2.56-8.79). , and subgroups by location where a statistically significant association was also found in metropolitan Lima (OR=7.64; 95% CI=1.28-45.59) and on the Coast (OR=6.37; 95% CI= 3.17-12.78). DISCUSSION: The limitations of the study were that there were few studies analyzed, which reflects a low population representativeness, the design of the studies were different so there was a high risk of bias and high heterogeneity. Likewise, the measuring instruments for HAART adherence were not standardized for all the studies analyzed. Our main result is that illicit drug use is associated with poor adherence to HAART in adult patients with HIV-AIDS in Peru. OTHERS: This study was funded by both researchers.
