Objective: Determine the risk of preterm birth based on maternal age at Santa Rosa´s hospital Materials and Methods: Retrospective cohort study executed at Santa Rosa´s hospital during the years 2009 to 2019, which included adolescent, young and elderly pregnant women. The relative risk of preterm birth in pregnant women of these age groups was evaluated through Poisson regression with robust variance and adjusted model. Results: The study included 24,139 pregnant women who met the inclusion and exclusion criteria. The percentage of adolescent mothers was 14.57% and elderly mothers was 13.45%; and the incidence of preterm delivery was 6.86%. In the bivariate analysis, age, age group, marital status, province, parity, pregnancy control, urinary infection, hemorrhage due to uterine atony, premature rupture of membranes, type of delivery, APGAR at 1 minute and 5 minutes, weight, height and head circumference were statistically significant (p <0.05) for preterm delivery. In the adjusted multivariate analysis, the children of older pregnant women and pregnant adolescents had a higher risk of preterm birth than the children of young pregnant women (aRR= 1.37; 95% CI 1.20-1.56; p< 0.001; aRR=0.98, 95% CI 0.85-1.13, p<0.76, respectively). Conclusions: The relative risk of preterm birth of the children of pregnant adolescents and pregnant elderly women was determined, they have a higher risk of being a preterm birth compared to the children of young pregnant women.
