Objective: To evaluate the association between adherence to highly active antiretroviral therapy (HAART) and health-related quality of life (HRQoL) in people living with Humans Immunodeficiency Virus infection (PLHIV) controlled by demographical, clinical and psychological variables. Methods: We carried out a cross-sectional study in patients receiving outpatient medical care in the Infectiology department of a general hospital of Social Security in Peru. We assessed the adherence to HAART using CEAT-HIV and the quality of life by WHOQOL-HIV BREF. In addition, we measured age, sex, sexual orientation, depressive and anxiety symptoms, fear of COVID-19, comorbidities, and HAART scheme. We estimated crude prevalence ratios (RPc) and adjusted prevalence ratios by statistical (Rpa1) and epidemiological criteria (RPa2) with 95% confidence intervals (95%CI) using generalized linear models of the Poisson family models with non-parametric bootstrap with corrected and accelerated bias of 1000 repetitions. Results: We included 323 patients, 23,2% were classified as adherent and 35% had impaired HRQoL. The frequencies of impaired HRQoL were 9,3% in adherent and 42,7% in non-adherent patients (p<0,0001). Regression models showed an association between non-adherence and impaired HRQoL in crude (PR=4,58; CI95%:1,35-15,56) and adjusted models by statistical criteria (Rpa= 3,59; CI95%:1,02-12,7) and epidemiological criteria (Rpa=3,88; CI95%:1,74-8,66). Conclusions: Non-adherence to HAART increases the probability of HRQoL impairment in People living with Human Immunodeficiency Virus, regardless of demographic, socioeconomic, psychological, and clinical factors measured.
