Introduction: Mental health is defined as a state of emotional, cognitive and behavioral balance that allows the individual to function responsibly in their family, social and work environment, as well as enjoy well-being and quality of life. Resilience is a process of adaptation to a situation of great adversity with positive results, while academic adaptation refers to the process of adaptation of the student to the new learning conditions. The COVID-19 pandemic has affected the mental health of people worldwide and young university students have had to learn to adapt. Objective: to qualitatively synthesize the evidence in the literature about the self-perceptions of university students about their mental health and adaptation to university life in a pandemic situation. Methods: Qualitative systematic review of the literature published during the period 2020 to the present. An electronic search was performed in EBSCO, MEDLINE, OVID, ProQuest, SCOPUS, LILACS, PubMed, SciELO and Virtual Health Library databases; and 12 studies were identified that assessed college students' self-perceptions regarding their mental health and academic adjustment. Subsequently, data were extracted and synthesized using thematic synthesis as a qualitative synthesis methodology. The confidence of the findings was assessed using the GRADE – CERQual approach. Results: Twelve studies were analyzed, of which 7 were from high-income, 2 middle-income, and 3 low-income countries. In the studies, explore the opinions and perceptions of college students regarding their experiences in their academic adjustment and mental health during the COVID-19 pandemic. The review identified the main problems presented by students during the COVID - 19 pandemic, classifying them in mental health, academic adaptation and resilience. Conclusions: Based on the narratives of the participants in the studies that, according to this review, it is described that the mental health of university students was affected during the COVID-19 pandemic, due to the restrictions proposed by world governments leading students to a quick transition and new student virtual learning. Therefore, this review shows the adaptation and academic resilience that university students experienced during this period.
