Introduction: According to the WHO, 30 to 50% of couples that present difficulties conceiving, the defect comes from the male, and the best method to analyze the its reproductive capacity is the spermiogram. There are very little investigations about the effect geographical environment has on this parameters. Objective: To identify the presence of association between sperm parameters and exposure to high geographical altitude in three Peruvian regions. Methodology: This is an analytical cross-sectional study with non-probabilistic sampling by convenience in Lima, Arequipa and Cusco. The exposure variable comes from a self-report and the result variable from the laboratory report. For the multivariate analysis, generalized linear models were used, with the Gaussian family. Results: We gathered 744 participants, 336 were obtained in Lima, 329 in Arequipa and 79 in Cusco; therefore, 49.60% had no exposure to height, and 50.40% had exposure to high altitude. In the multivariate analysis we found an statistically significant association between seminal volume and exposure to high altitude (Exp (b): 0,755; IC: 0,596-0,958; p: 0,021); regard sperm concentration, sperm motility and normal sperm morphology, the study could not establish an association with the exposure to high altitude. Discussion: An association was found between the seminal volume and the exposure to high altitude, seen in the volume and sperm concentration, explained by the physiological stress secondary to hypoxia in high altitude areas. The most important limitations were a biased population, and not being able to have an adequate control of the exposure variables. Conclusions: An association was found between residence height and two of the sperm parameters. This finding sets a new precedent to expand future research in the reproduction field and physiological adaptation to altitude, especially in this nation.
