The problem addressed focuses on the lack of planning and specific measures to confront the Covid-19 pandemic in Peru. The situation intensified due to the lack of a contingency plan, which led the Cayetano Heredia National Hospital to take measures on the fly. Dr. Raúl Acosta led the “Covid Command“ and made crucial decisions, standing out for implementing modular, improving online services and engaging medical professionals. Key elements include Dr. Acosta's leadership, modular implementation to increase capacity, improvements in online services, and physician engagement. Statistical data supports the relevance of the problem, such as the 90% increase in telemedicine and 4,310 requests for remote medical care in the first weeks of the service. Data collection was carried out through research sources, interviews with directors and decision makers, and documents such as reports, theses and statistics were used. In the theoretical-conceptual framework, antecedents related to the management of health crises were reviewed, highlighting the lack of planning in previous situations. Strategies implemented by hospitals and clinics were mentioned, as well as the importance of infrastructure. Key concepts such as “management” and “efficiency” were identified as critical to addressing the situation. Theories such as “Kotter's 8 steps,“ Dee Hock's “chaordic leadership,“ and “situational leadership“ were explored. The methodological approach chosen is qualitative, based on the experience of those involved and external sources. The justification for the relevance of the problem focused on the lack of planning, with internal and external factors contributing to the negative impact. The situation led to infrastructure improvements and an approved contingency plan for future crises. The functional areas were affected by the lack of a specific department, but the “Covid Command“ led by Dr. Acosta assumed that role. The proposed alternatives focused on the training and mobilization of all medical personnel, improvements in facilities and patient monitoring. In summary, Hospital Cayetano Heredia faced significant challenges due to a lack of planning for the pandemic, but leadership, commitment, and adaptability allowed for improvements and learnings for future crises.
