Introduction: Chronic non-communicable diseases are a public health problem, highlighting glucose metabolism disorders. It is known that the use of oral contraceptives (OCs) can alter said metabolism; however, many studies are inconclusive. The objective was to evaluate if women of childbearing age who use COCs compared to their non-use have differences in serum glucose, insulin and/or HOMA-IR levels. Methods: Systematic review following the Cochrane guidelines that included studies that answered the PECOS question. The MEDLINE, LILACS, Cochrane, Scopus, and Web of Science databases were used. The risk of bias assessment was made using the “New Castle Ottawa” and “JBI Critical Appraisal Checklist for Quasi-Experimental Studies” scales. Meta-analysis was performed using randomized models according to the Dersimonian-Laird method. Results: 2626 articles were found, of which 63 were included. All studies had low risk of bias. In women with polycystic ovarian syndrome (PCOS), there was a marked decrease in HOMA-IR: -0.45 (95% CI -0.74; -0.15) after using COCs. In women without PCOS, fasting insulin levels increased by an average of 5.57 pmol/L (95% CI 2.25, 8.89), and the HOMA-IR increased by 0.26 (95% CI 0.02, 0.50). There was no variation in glucose values for the two groups. The meta-analyses had moderate/high heterogeneity. Conclusion: In women without PCOS, insulin levels increased after using COCs; whereas, HOMA-IR levels decreased in women with PCOS and increased in women without PCOS. No effect on serum glucose was detected.
