Background: Since its discovery in 1981, HIV has remained a global public health concern. Access to HIV treatment is critical for preventing progression to end-stage AIDS and maintaining an undetectable viral load. However, the health disparities are greatest in Latin America and The Caribbean, which puts HIV-positive people at risk. Objectives: To determine the magnitude of inequalities in access to antiretroviral treatment coverage for people living with HIV/AIDS (PLWHA), according to the socioeconomic determinants in countries of Latin America and The Caribbean between 2000 and 2020. Material and Methods: Ecological study. The World Bank database's antiretroviral treatment coverage indicators (HAART) were used. The indicators of 24 countries were analyzed, including variables related to economic policy, work, urbanization and health services. Regarding data analysis, the absolute and relative inequality, the inequality gradient, and the health concentration curve and index (ICS) were calculated. Results: the inequality gap in access to antiretroviral treatment coverage in Latin American and The Caribbean countries, remained present, with minimal changes throughout the period studied. The absolute Kuznets gap and the relative Kuznets gap decreased in all the indicators studied in the last 20 years. Similarly, ICS also decreased in all the socioeconomic stratifiers. Conclusions: Reasonable social policies sensitive to health equity, such as targeting the most vulnerable and disadvantaged, can have a positive impact on access to HAART in Latin America and the Caribbean (LAC) despite the limited evidence for cost-effective interventions to address the social determinants of HIV. With this study, it is shown that there is still health disparities related to access to antiretroviral treatment coverage, and it is hoped that it will motivate researchers to determine the true inequality in LAC countries during the pandemic of COVID-19 in the years 2020-2021 and assess whether it is possible to eradicate the HIV/AIDS pandemic by 2030 as proposed by UNAIDS.
