Introduction: In Latin America, Peru is one of the countries with high levels of childhood anemia, being in 2019 the most affected regions, the mountains and the jungle. For this reason, it is important to evaluate the factors that affect adherence to iron treatment. Objective: To determine the association between beliefs and low adherence to iron treatment in children under 2 years of age. Methods: Cross-sectional analytical study between September and October 2021. A sample of 163 participants who attended the CRED service at CS Zarzuela-Cusco was included. The MMAS-8 instrument was used to measure therapeutic adherence and the BMQ survey (beliefs dimension) was used for caregivers' beliefs. We performed the appropriate analyzes including Poisson regression to calculate crude and adjusted prevalences. Results: The average age of the children was 13.12 months and that of the caregivers it was 29.64 years. 53.37% had low therapeutic adherence and 66% presented barriers in beliefs. Those caregivers with barriers in their beliefs increase the probability of low adherence by 1.58 times (95%CI=1.10-2.28). Likewise, for each year that the age of the child increases, the probability of low adherence increases by 3% (RPa=1.03;CI95%=1.00-1.07). Conclusions: An association was found between the caregivers' beliefs related to treatment and a low level of therapeutic adherence. It is suggested to consider the evaluation of the beliefs in the office with the purpose of identifying probable children who may present low adherence to iron treatment.
