Objective: To evaluate the efficacy and safety of sulforaphane for the reduction of symptoms in people with autism spectrum disorder. Methodology: Systematic review of randomized clinical trials published until April 1, 2022 in Medline, CENTRAL, Web of Science, LILACS, and gray literature sources. The combined effects of sulforaphane on general, behavioral, and socialization symptoms at 7, 14, and 22 weeks were evaluated by meta-analysis in Stata v. 16.1. Risk of bias was assessed using Cochrane's Risk of Bias (RoB). Results: Three clinical trials were identified. Compared with placebo, sulforaphane did not produce significant differences in most of the domains evaluated, with the exception of a beneficial effect observed at 7 weeks in socialization and at 14 weeks in behavior. Side effects were mainly mild gastrointestinal symptoms. Most of the studies had a low risk of bias in detection, performance, reporting, and random sequence generation. Conclusion: Sulforaphane produced transient improvements in socialization and behavior, without improving the general symptoms of autism or producing serious adverse events. However, additional studies with a larger number of participants, longer follow-up, in diverse populations and using standardized methodologies are required to confirm these findings.
