Introduction. Worldwide, cervical cancer is a major public health problem. In Latin America and the Caribbean (LAC), it is the third most common cancer among women and is the leading cause of female death in several countries. Furthermore, it is the main reason for years of life lost (due to cancer) in women. Objective. To demonstrate the magnitude of the inequality of disease burden distribution using incidence rates and disability-adjusted life years (DALY) of cervical cancer in LAC. Methods. An ecological study was carried out at the country level. Random-effects regression model was used to analyze the relationship between social determinant indicators (GDP, GNI, and life expectancy at birth) and cervical cancer incidence rates; as well as the relationship with DALYs for cervical cancer in 30 LAC countries between 1995-2019. Then, standard gap and gradient metrics of social inequality in cervical cancer incidence rates and DALYs were found across countries in 1995, 2000, 2005, 2010, and 2019. Results. The inequality regression curves showed a non-linear inverse gradient between social position -defined by life expectancy at birth, GDP or GNI- and the incidence rate; as well as with DALYs for cervical cancer. In addition, the concentration curves of inequality in health showed that the incidence did not decrease through the years. Conclusions. LAC countries with lower life expectancy at birth, lower GDP and lower GNI had higher incidence rates and DALYs for cervical cancer compared to richer countries.
