Background: In recent years, it has been documented that substance addiction has become a multifactorial problem that affects different sectors of society, and the prison population is one of the groups most predisposed to suffer from this disorder. This research aims to evaluate which characteristics of the prison population are associated with the self-report of this pathology. Methods: The present study is an observational, analytical, cross-sectional study based on the secondary analysis of the database of the National Penitentiary Census Peru 2016. The study population is made up of 76 180 inmates from the 66 penitentiary establishments in the country and the Callao naval base, who participated in the Censo Nacional Penitenciario Perú 2016. Results: The prevalence of self-reported substance addiction diagnosed after hospitalization was 0.25%. Associations were found, without establishing the directionality of causality, with diagnoses of TB (p=0.003), HIV (p<0.001), Depression (p=0.012), Anxiety (p<0.001), and Psychosocial Disability (p<0.001). There is association with variables that were already present before admission such as overcrowding(p=0.009), psychoactive drug use/frequency(p<0.001), tobacco use/frequency(p<0.001), parental alcohol use (p=0.027), and parental psychoactive drug use (p=0.017). Conclusions: Based on the results, strategies or interventions can be proposed to control this problem at different levels of prevention. Therefore, further studies are recommended to evaluate possible interventions and to monitor the effects of these factors after release from prison.
