Introduction: Globally, in 2019, the prevalence of decreased visual acuity at all ages was 2.2 billion people. In children, it was 19 million. The decrease in visual acuity is associated with several factors as malnutrition. “Young Lives” is a cohort that followed children for 15 years in four countries. Our goal is to assess which perinatal, neonatal, and early childhood factors are associated with decreased visual acuity in Peru. Methods: Cohort-type study, based on the secondary analysis of the “Young Lives“ study carried out between 2002-2016. We only included Peruvian children for the younger cohort (having 6-18 months at baseline in 2002) who had visual acuity evaluation by means of Snellen chart in 2009. Two outcomes were evaluated: decreased visual acuity of any severity (<20/40), and severe impairment (<20/200). Crude and adjusted relative risks (aRR) were obtained using Poisson Regression corrected by clusters. Results: 1907 children were included, 22.5% had decreased visual acuity, and 5.1% had severe impairment. Decreased visual acuity of any severity was associated to C-section birth (aRR 1.21; 95%CI 1.00-1.46) compared with vaginal delivery at hospital. In the adjusted model, rainforest region, rurality, poverty, birth at home, birth weight ≥4000 gr, and having ≥2 siblings decreased the risk. For severe impairment, rainforest region, rurality, and birth at home, were protective. Conclusions: Having been born by C-section, increased the risk of impaired visual acuity in general, but not severe. Variables as living in the rainforest, poverty, rurality, birth at home, and having large families decreased the risk. The underlying reasons for these effects should be further explored.
